In compliance with the recent changes under the Employer Reporting Improvement Act, we are informing you that Form 1095, which provides information about your health coverage, will no longer be mailed automatically. Instead, it is available upon request.

We are committed to processing all requests promptly and will provide the form within 30 days of your request or by January 31, whichever comes later.
